Heart is the climactic standard organ—pulsating chambers, vein networks, and the highest enemy density before optional secret content. Some routes force one-way vein travel where map fast travel (M) disables until branch completion, making reward choices permanent. Soft-body drift along veins can carry you into damage unless you counter-steer with Flagella.

Combat Escalation

Macrophage swarms combine with elite T-cell crossfires and adaptive immune debuffs simultaneously—priority targeting matters more than raw DPS. Rival parasites launch coordinated ambushes in rest-node adjacent rooms. Use shops before final boss approach if map allows backtravel.

Overcharge windows shorten on high Antibiotics—Mitochondria count and duration Traits define whether bursts feel impactful.

Atrioventricular Node Boss (A/V Node)

The Heart finale boss is the Atrioventricular Node—a multi-phase encounter that gates both standard run completion and Brain route access. Phase one typically features add-heavy macrophage spawns; phase two introduces beam lattices covering most of the arena floor; phase three compresses into melee-range desperation patterns. The July 17, 2026 hotfix made arc patterns more forgiving—update Steam before blaming difficulty on build failure.

Brain hunters must defeat the A/V node without taking the normal End Game exit when carrying four yellow puzzle shapes. Standard Heart victory triggers run completion rewards and plasmid fragment bonuses. Brain route players solve the shape puzzle instead of ending—opening the hidden passage leads to full Brain combat, not a credits screen.
