Where to Find Customization

Update 4 added manual difficulty customization in the main Settings menu, separate from per-run Antibiotics tiers chosen at run start. Open Settings before or between runs to adjust baseline enemy health, heart drop rates, and optional hazard toggles without reinstalling mods or editing save files.

This global difficulty slider complements—not replaces—the Antibiotics multiplier that scales rewards and elite density during individual attempts. Think of customization as your personal comfort floor: Easiest helps new players learn Skin and Intestine, while Normal remains the default benchmark for community tier lists and daily leaderboard comparisons.

Accessibility options stack with customization. Auto-shooting, gamepad-and-mouse hybrid input, and performance presets (Environment Quality Mid, Postprocessing Mid from Update 2) sit alongside difficulty so players can tune feel without sacrificing readable combat telegraphs.

What Each Tier Changes

Normal is the developer-intended default. Heart organ bosses, Lung second-phase patterns, and secret level gauntlets assume Normal health pools and standard heart healing drops between rooms. Use Antibiotics—not Easiest—for endgame farming once builds stabilize. Challenge modes and Photosynthesis runs typically stay on Normal baseline so mutator rules remain meaningful.
